Mission Statement:

Provide empowered learning and coping through an individualized journey involving support from family, peers, medical staff and community resources in addition to a personal vision. This will enable participants to have hope and confidence to self manage diabetes as part of their whole wellness.

 

Program Goal:

Provide a participant-directed diabetes self-management education program.

 

The PTHA Diabetes Team is available to answer questions and provide the support you need to live a long, healthy life with diabetes. It’s your health – Take Charge!
